About the Role

Leighton Linslade Health Connections PCN are looking to recruit to the post of social prescriber link worker, to work within our developing Primary Care Network multidisciplinary healthcare team.

Our Social prescribing link worker will connect people with local community activities and services that can help improve their health and wellbeing. A vital part of your role will be to build relationships with the people you are helping by listening carefully to what’s important to them and what motivates them.

Social prescribing empowers people to take control of their health and well-being through referral to non-medical ‘link workers’ who give time to focus on what matters to each individual, taking a holistic approach and connecting people to community groups and statutory services, for practical and emotional support.

You will empower people to take control of their health and wellbeing, focus on 'what matters to them’ and take a holistic approach to an individual's health and wellbeing, connecting people to diverse community groups and statutory services for practical and emotional support.

You will be a key part of the primary care network (PCN) multi-disciplinary team.

You will work closely with GPs and practice teams, making sure that appropriate support is made available to them; enable people to access the services and support they require to meet their health and wellbeing needs, helping to improve people’s quality of life.

You will work with a diverse range of people from different cultural and social backgrounds and have the ability to work confidently and effectively in a varied, and sometimes challenging environment is essential.

About the Candidate
If you are a local community champion or already a social prescriber, and want a new and exciting challenge, then we would love to hear from you.
 
You will need to have the following skills :
  • Experience of working within healthcare, the voluntary or community sectors, supporting vulnerable groups;
  • NVQ Level 3 in Health and Social Care, or equivalent qualification or experience;
  • Understanding of the wider determinants of health, including social, economic, and environmental factors and their impact
  • Strong listening and communication skills;
  • Knowledge of IT systems, including ability to use word processing skills, emails, and the internet to create simple plans
  • You will be familiar with local resources and services and how to access them;
  • Motivated by helping people, with care and empathy;
  • Ability to communicate effectively, both verbally and in writing, with people, their families, carers, community groups, partner agencies and stakeholders
  • Ability to deliver person-centred support in a non-judgmental way;
  • Experience of completing holistic, person-centred care-planning assessments;
  • Understanding of the needs of small, volunteer-led community groups and the ability to support their development;
  • Commitment to collaborative working with all local agencies;
  • Experience and confidence in the delivery of lifestyle change interventions.

Leighton-Linslade Health Connections is the primary care network for Leighton Buzzard and works with the three practices in Leighton Buzzard serving a population of c51,000.

We offer various primary care support roles to help the population take ownership of their health and connect with the community; working with the Personalised Care Model to help patients achieve their goals and focusing on what is important to them.
