About the Role

Green Vale Health Primary Care Network is a newly formed PCN, with an opportunity for a physiotherapist to join our team on a full-time basis.

The role will involve working with GPs and other clinicians, to identify and manage a caseload of patients, playing a crucial role in supporting the patients and their carers both in the surgery and in their homes, ensuring that their changing needs are addressed.

The main duties include:

  • Being non-judgemental, empathetic and supportive, as the first point of contact for patients, clinicians and community carers.
  • Assisting in the planning and organising the full range of primary care physiotherapy.
  • Service activities, inclusive of workload delegation and reallocation, as required and dependant on staff resources.
  • In conjunction with colleagues, leading the development of primary care physiotherapy services, including direct access to physiotherapy in primary care following national and local guidance including NICE.
  • Identifying problems which effect planning and delivery of patient care and actively work towards solving them empowering others to make decisions.
  • Maintaining an area of clinical expertise, so providing a highly specialist clinical resource for the specialty or take a lead in a specific area of designated responsibility within the specialty.
  • Communicating with carers, patients, referrers and other professions to ensure the provision of excellent patient care. This may at times require staff to overcome barriers to understanding (impaired hearing, speech or cognition) and could include the transmission of sensitive information. This requires an empathetic approach to patient care.
  • Ensuring the effective and efficient use of physical and financial resources and make decisions regarding supplies and equipment within specialist area in conjunction with all other therapists and team.
  • Delegating work to match capabilities and workload of therapy team members, in discussion with all team members.
  • Occasionally responding to bleep and mobile for service clinical practice.
  • Providing expert care for either a defined case load of patients or offer highly specialist service.
  • Providing advice to patients within agreed protocols and clinical guidelines, but also has the ability to make decisions outside these frameworks where necessary based on best practice.
  • Performing advanced clinical skills in the assessment of patients including taking a clinical history, performing a clinical examination and evaluating the clinical information, to produce an appropriate plan for the implementation of care delivery, whilst maintaining accurate patient records.
  • Performing therapeutic procedures and investigations related to specialty, that require dexterity, coordination and precision to perform appropriate assessment and interventions. Examples include therapeutic handling techniques manual techniques, balance and exercise prescription and falls prevention strategies such as getting up off the floor.
  • Providing information and advice on prescribed or non-prescription medication, on medication regimens, side-effects and interactions.
  • Integrating appropriate non-drug based treatment methods into patient management plans.

About the Candidate

If you are a skilled MSK physiotherapist, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

The ideal applicant will have had the following skills, experience and qualifications:

  • Experience of working in an NHS or social care organisation.
  • Passed training requirements as outlined by the Personalised Care Institute (or be prepared to enrol and complete next available course)
  • Previous experience of working in a customer-focused environment
  • Previous experience of working towards targets/deadlines
  • Access to own transport and/or ability to travel across the locality on a regular basis, including to visit people in their own home.
  • Strong listening and communication skills.
  • Motivated by helping people, with care and empathy.

Some flexibility is required, as you will sometimes need to work alternative hours to cover for colleagues within the PCN.


About Us

Green Vale Health Primary Care Network is a newly formed PCN in central Bedfordshire. Our mission is to improve health and care and to meet everyday health and care needs for people by connecting primary care systems and using creative thinking to develop, improve and support great local services.

Our PCN is split over two sites, Flitwick Surgery and Marston Forest Healthcare, both are modern facilities. We have a combined list size of approximately 25,000 patients with some 243 care home beds. We are not one of the larger PCNs, but this smaller patient number allows us to provide first class healthcare and gives us the ability to target diverse healthcare needs.

The sites are only a short distance apart (5 miles) but service the main population areas of Flitwick, Marston Moretaine and Cranfield, as well as several surrounding villages in a rural setting, with Bedford, Milton Keynes and Luton only a few miles away.

Being a newly formed PCN we are looking to recruit staff through the ARRS funding stream to work with our existing staff to further develop the services we can offer our patients. All our vacancies are compliant with the standard NHS terms and conditions of employment, and offer inclusion in the NHS pension scheme. Salaries are dependent on skills and experience, but we would expect them to be within the AfC bandings for the role.
