Church Hill Surgery has a fantastic opportunity for an experienced practice nurse to join our small, friendly and established s team on a part-time basis, for 18 hours per week covering Wednesday and Friday, 08.30 - 18.30, on a 1 year fixed term contract.
You will be supported by a friendly and caring clinical team of doctors, advanced nurse practitioner, practice nurse and health care assistants, as well as the practice team. The role will involve visiting patients in their residential home for ongoing care, as part of our outreach service.
The main responsibilities of the role include:
- Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
- Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met.
- General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
- Managing and treating long-term conditions, in line with national and local policies and the practice needs.
- Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
- Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
- Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
- Meeting the needs of patients presenting for opportunistic wound care.
- Hypertension review and management.
