About the Role
Greensand Surgery has a fantastic opportunity for a experienced long term conditions nurse to join our team on a part-time basis, for 18 hours per week. 
 
As a specialist nurse, you will provide and maintain a high standard of chronic disease management, which includes diagnosis, monitoring & development of individual management plans. Specifically in Diabetes, CHD and HF you will work strictly in accordance with specific practice guidelines & protocols.
 
You will work collaboratively within the multi-disciplinary general practice team, to meet the needs of patients, provide assessments and reviews of health care needs, the optimisation of care where co-morbidity exists, maximising wellbeing, self-empowerment, and quality of life. 
 
The main responsibilities of the role include:
  • Responsible for management of long-term condition clinics in diabetes, chronic heart disease & heart failure.
  • Recognise health promotion opportunities, give brief, focused lifestyle advice using motivational interviewing techniques which may include alcohol screening & smoking cessation.
  • Complete annual long-term condition reviews for patients.
  • Complete medication reviews, change when necessary according to results.
  • Review patients who present to the service on the day needing an acute service.
  • Process pathology, workflow & tasks.
  • Be proactive in improving and influencing the patient care pathway/ journey.

About the Candidate
All applicants should have the following qualifications, registrations, skills and experience:
 
Essential:
  • Registered nurse.
  • NMC 1st level registration.
  • Accredited diploma (or above) level award in at least one chronic disease e.g. diabetes, CHD or HF.
  • Experience of continued professional development.
  • Experience in management of long-term conditions specifically CHD, HF and Diabetes.
  • Experience in primary or community nursing care.
  • Have an understanding of QOF and enhanced service management.
  • Willingness to undertake training and education.
  • Willingness to improve and develop the patient experience and patient care pathway.
  • Experience of using Microsoft software.
Desirable:
  • Non-medical prescribing qualification (or willing to work towards).
  • Experience of SystmOne and using Arden's templates.
  • Involvement in implementing and using protocols and clinical guidelines.

Greensand Surgery is a semi-rural, GMS, practice in mid-Bedfordshire with just over 10,000 patients.

We are a dynamic, hard-working team whose aim is to provide high quality patient centred care. The partnership is supportive and cohesive and we have a full complement of enthusiastic team members. We provide a comprehensive range of medical services for our patients as well as meeting targets for the New Contract. The practice consists of three partners and four salaried GPs, a dedicated nursing team, clinical pharmacist, and admin staff.
