Advanced Nurse Practitioner/Advanced Clinical Practitioner (Paramedic)

Hoveton and Wroxham Medical Centre is looking to recruit an advanced nurse practitioner/advanced clinical practitioner to join our multi-disciplinary team. This role is to provide acute "on the day" appointments as well as pre-booked appointments for patients with acute illness or ongoing problems.  

• Treat a variety of acute and chronic presentations.
• Work independently to see patients in the surgery, in care homes and housebound patients.
• Work well under pressure and as part of a team delivering high quality care.
• Prescribe independently, safely and effectively.

The post-holder will:

• Assess, diagnose and treat undifferentiated patients with a range of acute, non-acute and chronic medical conditions and minor injuries.
• Initiate appropriate care plans including requesting necessary tests, onward referral to other health care professionals and agencies and follow up consultations.
• Promote health and screening in relation to primary prevention strategies.
• Facilitate innovative approaches to effective systems and processes for patient management.
• Work with the clinical and management teams to deliver a timely, cost effective service.

• Professional registration with relevant professional body: e.g.NMC
• Post-graduate qualification in Advanced Clinical Practice

• Sound clinical reasoning & decision making
