About the Role

Woodview Medical Centre are looking for an enthusiastic and motivated diabetes nurse to join our team and take a leading role in the management and education of patients with diabetes across our practice population. This position is offered on a part-time basis for 1 day/2 sessions per week. 

As our diabetes nurse, you will:

  • Work collaboratively with GPs and the wider clinical team to meet the needs of patients supporting the delivery of both drug and non-drug based treatment plans following local and national policies and procedures.
  • Run nurse-led diabetes clinics and support QOF achievement.
  • Assess, plan, develop, implement and evaluate treatment programmes that optimise the health and well being of patients in conjunction with the patient and/or their carers.
  • Deliver comprehensive diabetes care, including reviews and patient education.
  • Maintain effective communication with patients and/or carers to act as a first point of call for diabetes related issues.
  • Refer newly diagnosed patients to DESMOND
  • Request and interpret diagnostic investigations and inform patients of results
  • Discuss medication changes to achieve target HbA1c and lipid levels
  • Promote self-management, lifestyle modification, and preventative care.
  • Proactively identify and manage treatment plans for patients at risk of developing complications associated with diabetes and manage these patients.
  • Identify vulnerable groups and provide support, care and education to enable them to be self caring.
  • Monitor blood pressures and QRISK
  • Provide weight management advice and to discuss and plan weight management programmes with patients and refer as necessary
  • To be able to provide diabetic care for patients in their own home if required would be desirable.
  • Work in close liaison with specialist tissue viability and podiatry services where necessary.
  • Contribute to audits, service improvement, and the ongoing development of chronic disease management within the practice.

About the Candidate

We’re looking for a Registered Nurse (NMC registered) with experience or additional training in diabetes care. You should be passionate about improving outcomes for patients with long-term conditions and confident working independently within a supportive team.

Essential:

  • Experience in diabetes management and patient education
  • Excellent communication and organisational skills
  • Commitment to evidence-based practice and continued learning

Desirable:

  • Primary care experience
  • Diabetes diploma or equivalent qualification (MSc desirable but not essential)
  • Experience using EMIS/SystmOne
