About the Role

An opportunity has arisen for a clinical pharmacist to join South Uttlesford PCN, to provide high quality patient focussed pharmacy services to the patients of the PCN. This position is offered on a full-time basis and based in/out of a variety of South Uttlesford practices/locations according to need.

Main duties will include;

  • Patient facing long-term condition clinics to review the ongoing need for each medication
  • Patient facing clinical medication reviews
  • Patient facing care home medication reviews
  • Patient facing and telephone medicines support
  • Unplanned hospital admissions – review of medications and reconciling of medications
  • Repeat prescribing
  • Identification of high risk patients

About the Candidate

You will be a qualified pharmacist with strong experience within primary care and have the following skills and qualifications:
 
  • As a minimum, you will hold a degree in Pharmacy (Masters or equivalent)
  • Be registered with the General Pharmaceutical Council and a member of the Royal Pharmaceutical Society
  • Ideally you will be a non-medical independent prescriber but the PCN will actively support you to achieve this, if not already an independent prescriber
  • You will be required to lead by example, setting and maintaining high standards of professionalism within a team environment
  • You should be IT literate and confident in using practice systems
