About the Role

North Colchester PCN (Primary Care Network) have an exciting opportunity for a care coordinator to join us in assisting to support primary care communications across our PCN. This role is offered on a part-time basis for 30 hours per week working Tuesday – Friday each week. This is a fixed term contract until the end of March 2027. 

The role involves working very closely with the PCN Business Manager and Clinical Director, whilst building relationships with admin teams across the PCN. You will work with the Multi Disciplinary Team (MDT) within the PCN. The role is pivotal in ensuring the smooth running of the PCN and ensuring suitable training materials and resources are shared in an appropriate manner. This position forms part of a wider Care Coordinator team model in which individual roles may hold defined areas of operational focus, while maintaining shared core responsibilities and a strong MDT approach

Duties will include;

  • Collate and develop promotional material to improve uptake of existing and new technology
  • Support the PCN team with any relevant projects and tasks
  • Support with sourcing relevant training material for the PCN wide team
  • Schedule meetings and manage meeting minutes
  • Assisting with HR administration, closely liaising with the PCN Business Manager
  • General administrative tasks in order to support across the PCN

About the Candidate

The ideal candidate will hold the following experience and key attributes:

  • Ideally you will have a Diploma Level 2 in Health and Social Care or equivalent qualification
  • Experience of working within healthcare or the voluntary/community sector, supporting vulnerable groups
  • A keen eye for detail
  • Proven experience of supporting a busy team with HR responsibilities
  • Experience in training and building training materials
  • Experience of SystemOne and EMIS
  • Strong listening and communication skills
  • Familiar with local NHS training resources and services and how to access them
  • Motivated by helping people, with care and empathy and in delivering person-centred support in a non-judgmental way
  • Strong administrative experience
  • Proficient in Microsoft Office
  • A willingness to develop the role
