We are currently seeking an experienced and motivated Practice Nurse with a genuine interest and experience in Diabetes to join a dynamic healthcare team based in the beautiful county of Suffolk. This is an exciting opportunity to deliver high-quality, specialist diabetes care while enjoying working in a purpose-built surgery.
This position offers a guaranteed 10-hour working week with more hours available if required on variable days to suit.
As a Diabetes Practice Nurse, you will provide expert, patient-centred diabetes care across the primary care setting. Working collaboratively with patients, carers, and multidisciplinary teams, you will play a key role in improving patient outcomes and promoting self-management.
The purpose of the role is to provide nursing services to the practice population including chronic disease monitoring, screening and advice within the professional competence of the post holder. The post holder will work at all times to the Code of Conduct, as set out by the Nursing and Midwifery Council.
