About the Role

South Rural Primary Care Network are seeking to recruit a PCN Patient Care Coordinator to be based at our Hadleigh Health Centre. This position is offered on a full-time basis. 
 
 Duties include; 
  • Assist the doctors and other clinicians in the provision of a high quality service.
  • Support the practice care home team in the delivery of this service to help manage people’s needs and coordinate the delivery of MDTs.
  • Work collaboratively with our care homes to support communication and the administrative work required for the practice to deliver this service
  • Support delivery of QOF, incentive schemes, QIPP, Investment and Impact Fund (IIF) and other quality or cost effectiveness initiatives.
  • Support the delivery of long term conditions clinics working with the clinical team to ensure patients are invited and we reach patients who require additional support and/or have learning needs.
  • Work collaboratively within the team to manage patients needing medication reviews and support the delivery of this work to ensure all patient groups are reached.
  • Support of the delivery of our vaccines contracts.Including stock take, ordering, reporting of activity.
  • Support with the Covid and Flu clinics including stock control across the PCN, planning the consent and vaccination of each of our care homes and housebound patients. Scheduling staff for the large onsite clinics and managing the booking system.
  • Work closely with health care professionals to ensure registers are kept up to date and appropriate reviews are completed for Palliative, Dementia, learning disability and serious mental illness.
  • Help design efficient templates, protocols, alerts and other IT tools to support safety, patient care, quality record keep keeping and reporting.
  • Manage cervical cancer recalls and the development of systems to help reach groups where uptake is low.

About the Candidate

If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

The ideal applicant will have the following skills, experience and qualifications:

Essential: 
  • Excellent communication skills (written and oral)
  • Flexibility to work outside of core office hours
  • A clear understanding of child protection policy and procedures and commitment to the safeguarding of children and vulnerable adults
Desirable: 
  • Experience of working in a primary care setting
  • Experience of working in a care home setting
