About the Role

The De Parys Group has an opportunity for a  patient services care co-ordinator to join our team on a full time basis (job share/2 x part time roles considered)

The post-holder will provide extra time, capacity and expertise to our practice team to help us to improve the care provided to our patients.

The main duties of the role include:

  • Attend multidisciplinary team meetings and will have a central role in organising those meetings with the clinicians and ensure appropriate follow up for patients occur.
  • Chair and create agenda for the multidisciplinary team meetings with other external professionals and to monitor and update the patient list. Then action any relevant points for the PCN.
  • Work closely with the ARRS funded roles and identify where a patient could potentially benefit from this service.
  • Work alongside our aligned care homes and co-ordinate Weekly Home rounds and ensure each patient has a Personalised care support plan, Man the Care team direct phone line and promptly actioning the requests to the relevant teams.
  • Organise and co-ordinate referrals as appropriate and liaise with other teams within the wider system to ensure holistic care is provided to all the patients.
  • Raise awareness of health promotion & screening such as NHS Health Checks, SMI and LD Health Checks in practices and coordinate invitations to ensure targets are achieved on a practice and PCN level.
  • Run Audits and searches where necessary to identify patients for review.
  • Support and raise awareness of national screening and immunisation programmes.
  • Support Impact and Investment funds framework (IIF) and quality and outcome framework (QoF) alongside the PCN DES requirements as well as additional PCN projects.
  • Supporting with best interest and safeguarding for individual patients.
  • Managing the Practice’s Subject Access Request (SAR) service
  • Coordinating Weight Management and specialist referral pathways
  • Support Adult and Child Safeguarding and Did Not Attend (DNA) coordination
  • Issuing and managing Medical Exemption Certificates
  • Coordinating cancer and urgent referral pathways
  • Coordinating hospital and secondary care follow up
  • Coordinating diagnostics, pathology, and testing

About the Candidate

If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

The ideal applicant will have the following skills, experience and qualifications:

  • Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups.
  • Diploma Level 2 in Health and Social Care, or an equivalent qualification (desirable not essential).
  • Familiar with local resources and services, including how to access them.
  • Motivated by helping people, with care and empathy.
  • Able to deliver person-centred support in a non-judgmental way.
  • Experience of completing holistic person-centred care planning assessments.
  • Strong listening and communication skills.
