About the Role

Rowhedge & University of Essex Medical Practice is currently looking for a Social Prescriber to join our team. The Social Prescriber will play a vital role in enhancing patient care and well-being by connecting individuals with non-medical support services within the community.

The post holder’s responsibilities will include:

  • Collaborate with healthcare professionals to identify patients who would benefit from social prescribing.
  • Conduct holistic assessments to understand patients' social needs and preferences.
  • Develop personalized care plans and signpost patients to relevant community resources and services.
  • Educate patients on available social support options and empower them to take control of their health.
  • Organize and facilitate group activities that promote mental well-being, physical health, and social inclusion.
  • Maintain accurate records of patient interactions and outcomes in accordance with data protection regulations.

 



About the Candidate

Experience in the following would be desirable:

  • Experience in promoting wellbeing services to facilitate independence personal resilience.
  • Experience in creating wellbeing assessments that take into consideration the role and possible impact of the wider determinants of health on wellbeing.
  • Experience in building relationships with key staff in GP practices , attending relevant meetings, becoming part of the wider network team, giving information and feedback on the role of social prescribing.
  • Have experience as a Health Coach
