About the Role
At St James Surgery in Clacton on Sea, we are looking to recruit a part-time practice nurse, qualified in diabetes management, to join our existing nursing team to work all day, 2 days per week - Monday & Tuesday.  
 
The main responsibilities of the role include:
  • Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
  • Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met.
  • General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
  • Managing and treating long-term conditions, in line with national and local policies and the practice needs.
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
  • Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Running diabetes clinics and reviews.
  • New patient and health checks.
About the Candidate
All applicants should have the following qualifications, registrations, skills and experience:
  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.
  • Competence to undertake diabetic clinics and reviews, holding relevant certificates.
  • Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work.  The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
  • Knowledge, and experience of its application, in supporting long-term conditions.
  • Insight and understanding of current issues in nursing and the wider NHS.
About Us
We are an EMIS Web practice with a patient list size of 14,400 patients. Our main surgery is in Clacton with a branch in St Osyth.
