Rochford Primary Care Network has a fantastic opportunity for a healthcare assistant - population health, to join the practice to support the PCN team, on a full-time basis.
Working under the direct supervision of the population health practitioner, the post-holder will work within their own competencies and will be expected to conduct clinical aspects of NHS Health Checks.
Main responsibilities include, but are not limited to:
- Conduct NHS health checks, new patient health checks & over 40 health checks.
- Conduct baseline observations such as pulse oximetry, blood pressure etc recording findings accurately.
- Conduct Body Mass Index (BMI) checks.
- Helping raise awareness of health & well-being & how it can be promoted, providing advice to patients.
- Supporting patients to adopt health promotion strategies and encourage principles of self-care.
- Deliver opportunistic health promotion & lifestyle advice.
- Supporting the practitioner with the management of chronic diseases in a community-based setting.
- Blood pressure and hypertension recording and assessment.
- Dementia screening and signposting.
- Give accurate & appropriate information to patients and groups within own competencies.
- Work to address health disparities within the community by advocating for equitable access to healthcare & public health resources.
- Assist in planning & executing health promotion campaigns & initiatives, including health screenings & awareness events.
