The Kelvedon & Feering Health Centre are seeking to recruit a full time practice nurse at our growing practice in the village of Kelvedon & Feering.
The ideal candidate will have experience of primary care nursing and able to perform a range of procedures in vaccinations, immunisations, wound care, ECG, phlebotomy, INR. A knowledge of LTC management (Diabetes, Respiratory Conditions, Asthma/ COPD, hypertension) will be an advantage. The post-holder will be comfortable using IT. Training will be given to improve and develop your skills. The practice has plans to move to a purpose-built site soon and we encourage staff to continue their PDP and attend training as required.
We are looking for a nurse clinical team lead to take the lead in infection control and who will collaborate closely with a team of supportive and collaborative clinicians to ensure the greatest level of care delivered to all patients.
The main responsibilities of the role include:
- Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
- Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met.
- General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
- Managing and treating long-term conditions, in line with national and local policies and the practice needs.
- Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
- Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
- Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
- Meeting the needs of patients presenting for opportunistic wound care.
- Hypertension review and management.
- Running asthma, COPD and diabetes clinics and reviews.
- New patient and health checks.
