About the Role
Bury St Edmunds Primary Care Network has an opportunity for a care co-ordinator to join our PCN, based at Guildhall & Barrow Surgery or Victoria Surgery, two of our 5 GP practices across the Bury St Edmunds area and be part of our senior clinical support team. This role is offered on a full-time basis, for 37.5 hours per week, over 5 days, primarily Monday to Friday between the hours of 0830 and 1800hrs, with some weekend hours required subject to demand.
 
In this pivotal role you will be working very closely with the practices and the multidisciplinary team (MDT) within the PCN, and is pivotal in ensuring that all patients receive the best possible care and service, working on requirements for the PCN Direct Enhanced Service Contract (DES).
 
The main duties of the role include:
  • Supporting the surgery in coordinating all key activity, including access to services/care navigation, advice, and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred.
  • Supporting digital initiatives.
  • Ensuring patients are invited and have access to appointments for vaccinations including flu and covid boosters.
  • Ensuring patients are educated around the different roles of health care professionals currently offering primary care services.
  • Supporting the surgery and patients during the long term condition review process.

About the Candidate

If you have experience of care planning, with strong communication skills and excellent organisational experience, then we would love to hear from you.

The ideal applicant will ideally have the following skills, experience and qualifications:

  • Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups.
  • Strong listening and communication skills.
  • Familiar with local resources and services, including how to access them.
  • Motivated by helping people, with care and empathy.
  • Able to deliver person-centred support in a non-judgmental way.
  • Excellent organisational skills with attention to detail;
  • Working within a team environment;
  • Respect patient confidentiality at all times;
  • Adaptable to the needs of the practice, willing to pitch in to get the job done!
