About the Role

Alexandra and Crestview Surgeries have a fantastic opportunity for an experienced nurse practitioner or advanced nurse practitioner to join our busy but friendly team on a full-time basis. As a nurse practitioner, you will take on a dedicated and important role. You will run telephone triage clinics in the surgery, as well as have a patient-facing role, you may also undertake home visits.

The main responsibilities of the role include:

  • Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
  • Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met.
  • General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
  • Managing and treating long-term conditions, in line with national and local policies.
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols.
  • Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
  • Meeting the needs of patients presenting for opportunistic wound care.
  • Hypertension review and management.
  • Running asthma, COPD and diabetes clinics and reviews.

About the Candidate
All applicants should have the following qualifications, registrations, skills and experience:
  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications;
  • Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.
  • Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
  • Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work.  The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
  • Knowledge, and experience of its application, in supporting long-term conditions.
  • Insight and understanding of current issues in nursing and the wider NHS.
About Us
Alexandra and Crestview Surgeries is a fantastic GP practice based over 2 sites with 15300 patients. We have a great support network both clinically and administratively. You will be joining our current team of 3 GP partners, 3 salaried GPs, 4 ANP/NPs, 5 practice nurses and 5 HCAa.  We are teaching and research practice. 
 
We are offering a good salary (depending on experience), NHS pension, annual leave and bank holidays (pro rata).  We are part of the Lowestoft PCN and also offer Extended and Improved access appointments outside of core hours.
