Care Coordinator

Little Waltham and Great Notley Surgery has an opportunity for a practice-based care coordinator to join our team on a full-time basis.

Being practice-based, the post-holder will be able to identify patients’ individual needs, ensuring that they are addressed by taking into account local priorities, health inequalities and population health management risk stratification.

The post-holder will have effective communication skills. A compassionate, caring approach is also essential for this post.
