Blackwater Medical Centre has a fantastic opportunity for a nurse prescriber/advanced nurse practitioner to join our team on a part-time basis for three days per week, hours can be flexible. This is a one year fixed term post starting mid March.
The successful candidate will work alongside a multidisciplinary general practice team to meet the needs of patients, support the delivery of policy procedures, and provide nursing leadership as required. The ANP should be able to manage clinical cases, deal with patient needs within a primary care setting and participate in nurse-led minor illness clinics.
The candidate should be able to demonstrate safe clinical decision-making and expert patient care within our practice. They should be competent in history taking, clinical assessment, diagnosis, and treatment and able to raise basic prescriptions.
Main duties of the job:
- Providing the choice of direct access to an NP, in the practice, to the community team and over the telephone, for the general practice population.
- Making professionally autonomous decisions.
- Providing a first point of contact for patients.
- Instigating necessary invasive and non-invasive diagnostic tests or investigations.
- Prescribing safe, effective and appropriate medication as defined by current legislative framework.
- Providing safe, evidence-based, cost-effective, individualised patient care
- Offering a holistic service to patients and their families.
- Promoting health and well-being through the use of health promotion, health education, screening and therapeutic communication skills.
- Referring patients directly to other services/agencies.
- Working as part of the practice team in developing and delivering specialist primary care services.
- Screening patients for disease risk factors and early signs of illness.
- Working with colleagues to ensure that National Service Frameworks are being delivered.
- Working with multi-disciplinary team to promote integrated and seamless pathways of care.
- Contributing to the practice achieving its quality targets.
- Participating in the identification of community health needs and develop strategies to address them.
- Helping develop and set up new patient services and initiatives to improve existing patient services.
- Being aware of and contributing to the planning and delivery of practice-based commissioning.
