About the Role
Thorpewood Medical Group located in Norwich, are looking for a diabetic specialist nurse to join our team on a full-time basis. 
 
Working as an autonomous practitioner, the role will encompass telephone triage, face-to-face consultations, home visits and interactions with the care homes linked with the surgery, with the focus of care on our diabetic patients.
 
The postholder will be required to work at the Thorpewood Surgery or Dussindale Branch surgery as required.
 
Main duties will include; 
  • Provide expert diabetes and nursing advice to others working within the team.
  • Assessment, diagnosis, treatment, prescribing, referral and discharge as appropriate and deliver key elements of surgery initiatives.
  • Instigate invasive and non-invasive diagnostic tests or investigations, interpretation of findings and reports.
  • Directly admit patients to secondary care outlets in medical need.
  • Refer patients to other medical specialities for assessment where indicated including fast track referrals.
  • Take responsibility for making autonomous decisions in relation to patient care.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
  • Break bad news to patients/parents/carers and provide appropriate ongoing support to meet identified needs.
  • Collect data and participate in regular practice audit and research projects.
  • Work effectively with individuals in other agencies to meet patients and carers needs.
  • Recognise and work within own competence and professional code of conduct as regulated by the NMC.
About the Candidate
All applicants should have the following qualifications, registrations, skills and experience:
  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Advanced clinical and diabetic knowledge/skills.
  • Competence in the use of IT systems, i.e coding work.
  • An understanding, acceptance and adherence to the need for strict confidentiality.
  • A can-do attitude.
