About the Role
The Tollgate Practice based in Colchester, are looking to recruit a lead practice nurse initially for a 3 month temporary post to start asap. This position is offered for 30-37.5 hours per week. 
 
We are a 2 partner practice looking after 10,000 patients and this key role is responsible for ensuring the delivery of safe and effective nursing care to our practice population. As the team leader for the general practice nursing team, the post holder is accountable for the nursing service delivery and will involve leadership training for our current nurse practitioner. You will work closely with our GPs and management to deliver the practice priorities.
 
Main duties:
  • Plan and deliver high quality integrated nursing care, promoting health and wellbeing within the practice population.
  • Work with clinical and non-clinical staff, other members of the wider healthcare team and NHSE/CCG in contributing to the achievement of QOF.
  • Deliver objectives set out in the NHS action plan meeting the standards as set out by the Care Quality Commission (CQC).
  • Oversee long-term condition clinics (including COPD, Diabetes, CHD).
  • Assess problems presented opportunistically by patients.
  • Manage and support the nursing team.
  • Support with leadership training within our nursing team
  • Manage and assist with the nursing team appraisals.
About the Candidate
Essential:
  • Available immediately/at short notice and happy to undertake a temporary role.
  • Registered general nurse, current NMC registration.
  • Previous experience in primary care. 
  • A demonstrable commitment to professional development.
  • Competency in nursing duties required for the post.
  • Excellent communication and interpersonal skills.
  • Ability to work well autonomously and within a team.
  • Experience of working in accordance with the NMC Code of Professional Conduct (2002) and The Scope of Professional Practice (1992).
  • Ability to work as part of an integrated multi-skilled team.
  • Able to work under pressure and in a changing environment.

We are a member of the Abbeyfield and East Hill Primary Care Network and within our locality sharing pharmacists, pharmacy technicians, physiotherapists, mental health workers, health and wellbeing coaches, and social prescribers, who all make a valuable contribution to providing excellent care to our patients.
