About the Role

North Colchester PCN (Primary Care Network) have an exciting opportunity for a permanent, full-time care coordinator to join us in assisting to support primary care communications across our PCN.

The role involves working very closely with the PCN business manager and clinical director. You will also work with the multi disciplinary team (MDT) within the PCN. The role is pivotal in ensuring the smooth running of the PCN and will include accessing services, advice, and information, and ensuring health and care planning is timely, efficient, and patient-centred.

We are seeking an individual with excellent person-centred planning experience, with local knowledge of key support services who can advise and support our patient population whilst having strong organisation skills. 

Key duties will include;
 
  • Support surgeries by generating reports using internal systems (full training will be given).
  • Support digital initiatives.
  • Take responsibility for the co-ordination of the patient’s journey through primary care.
  • Assist surgeries with recall processes (full training will be given).
  • Collate and develop promotional material to improve uptake of existing and new technology.
  • Support the PCN team with any relevant projects and tasks.
  • General administrative tasks in order to support across the PCN.

About the Candidate
The ideal applicant will have the following skills, experience and qualifications;
  • Experience of working within healthcare or the voluntary/community sector, supporting vulnerable groups.
  • Diploma Level 2 in Health and Social Care or equivalent qualification. (desired)
  • Strong listening and communication skills.
  • Familiar with local resources and services and how to access them.
  • Motivated by helping people, with care and empathy and in delivering person-centred support in a non-judgmental way.
  • Strong administrative experience, including experience of digital platforms/systems.
  • Proficient in Microsoft Office.
  • Eligibility to work in the UK, satisfactory references and DBS prior to employment.
