About the Role

Icknield PCN is seeking a high quality, experienced clinical pharmacist to lead our team of dedicated clinical pharmacists and pharmacy technicians, who work within our member practices. This is a full-time post to provide maternity cover for the lead clinical pharmacist. The ideal candidate will be an experienced clinical pharmacist with significant experience within primary care, and who is also an experienced manager or team leader.

Main duties of the job will include;

  • Support the effective delivery of patient care in GP practices within the PCN.
  • Utilise pharmaceutical skills to embed the principles of medicines optimisation within the wider practice teams.
  • Ensure safe patient care through regular review and monitoring of patients based on clinical need.
  • Support demand management through consultation with patients as appropriate.
  • Provide clinical leadership on medicines management and quality improvement.
  • Manage some aspects of the Quality and Outcomes Framework (QOF), Enhanced Commissioning Framework (ECF), Improvement and Investment Fund (IIF) and Enhanced Services.
  • Provide supervision, mentorship and line management to the appointed clinical pharmacists, pharmacy technicians and locum clinical pharmacists working across the PCN GP practices.

About the Candidate

You will be a qualified pharmacist with strong experience within primary care and have the following skills and qualifications:
  • Completion of a degree in pharmacy and registration with the General Pharmaceutical Council.
  • Post-qualification experience in pharmacy, and experience in primary care.
  • Qualified independent prescriber.
  • Member of the Royal Pharmaceutical Society (RPS).
  • Awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice.
  • Ability to demonstrate general pharmaceutical skills and knowledge in core areas.
  • Ability to plan, manage, monitor, advise and review general care plans for patients with long-term conditions.
  • Ability to demonstrate accountability for delivering professional expertise and direct service provision as an individuals.
  • Ability to recognise priorities when problem-solving and identify deviations from normal patterns, referring to seniors or GPs with confidence, when appropriate.
  • Ability to follow legal, ethical, professional and organisational policies, procedures and codes of conduct.
  • Ability to assist patients in decisions about prescribed medicines and supporting adherence as per NICE guidance.
  • Non-judgemental attitude and ability to be flexible and adaptable.
  • Self-motivated with excellent communication skills.
  • Understand the need for patient confidentiality.
  • Experience of managing or leading a team.
