About the Role

Central Basildon Primary Care Network is excited to be recruiting an advanced clinical practitioner to join our dynamic and supportive multi-disciplinary team. Offered on a part-time basis for 24 hours a week, ideally Tuesday, Wednesday and Thursday with flexibility, the successful applicant will work across 3 sites within our PCN. 

This role is to work as a PCN provider, contributing to the delivery of high-quality, coordinated care across our network. It is an excellent opportunity to play a key part in transforming and modernising local care pathways, helping us deliver patient-centred services across traditional professional boundaries.

The ideal candidate will;

  • Demonstrate a high level of expertise within their specialist area, providing advice, education and support to colleagues, patients, families and carers.
  • Practise autonomously as an advanced clinical practitioner, delivering comprehensive patient-centred care, including advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice.
  • Support innovative ways of working that enhance efficiency and patient experience, ensuring safe assessment, treatment, referral, and discharge of patients with undifferentiated or undiagnosed presentations.
  • Take a lead role in developing clinical practice and upholding high standards of care, including contributing to the creation and review of policies, procedures, protocols, and guidelines alongside multidisciplinary colleagues.    

This role offers you the chance to shape local services and make a meaningful impact within the community and work as a key PCN provider. 

About the Candidate

We welcome applicants from a variety of professional backgrounds. The successful candidate will be an experienced healthcare professional educated to Master’s level, with the advanced skills and knowledge required to work autonomously and confidently within an expanded clinical role. To be considered for this position, you will require the following: 
 

Qualifications, skills and experience required:

  • Professional registration with relevant professional body: NMC, HCPC or GPhC
  • Post-graduate qualification in specific speciality, or equivalent experience
  • Post-registration experience at a senior level (Band 7 or above) in specialty area
  • Evidence of on-going CPD
  • Advanced clinical assessment skills
  • Excellent communication skills
  • Ability to perform in stressful and complex situations
  • Sound clinical reasoning & decision making
  • Flexibility
