About the Role

Ludham and Stalham Green Surgeries are looking to welcome a care coordinator to their friendly team on a part-time basis, working approximately 17 hours per week. The agreed hours would need to be worked on a Monday and a Thursday each week. Please note this is a practice/PCN role, so you may be required to work at other NN4 practices.
 
This role would be key in providing and maintaining a high standard of care for patients health needs. The post holder will be an integral part of the general practice team, carrying out the following duties;
  • Oversee and coordinate patient requests via the digital Accurx system. 
  • Identify, where required and as appropriate, patient who need to be referred back to other health professionals within the PCN.
  • Provide coordination and navigation for people and their carers across health and care services.
  • Support patients to utilise decision aids, help create single personalised care and support plans, in line with best practice.  
  • Help people to manage their needs through answering queries, making, and managing appointments and ensuring that people have good quality written or verbal information to help them make choices about their care.  
  • Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing.  
  • Provide the interface between primary care and other services, helping patients with the transition between services.    
  • Attend and participate in any practice meetings when required

About the Candidate

If you are a skilled care coordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

The ideal applicant will have the following skills, experience and qualifications:

  • Minimum of 2 years experience of working with healthcare professionals and/or previous experience in the NHS or social care or relevant field. 
  • Experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice.
  • Ability to communicate complex and sensitive information effectively.
  • Ability to work under pressure and to meet deadlines.
  • Good IT skills.
