About the Role

Wootton Vale Healthy Living Centre are looking for a enthusiastic and motivated clinical pharmacist to join our friendly and supportive team on a full-time basis, flexible working considered. 

You will play a key role in medicines optimisation, long‑term condition management, and improving prescribing quality, while working closely with GPs, nurses, trainees, and the wider multidisciplinary team.

This is an excellent opportunity for a pharmacist who is passionate about patient care, enjoys clinical variety, and is keen to contribute to service development, teaching, and innovation within primary care.

Key responsibilities will include;
  • Undertake Structured Medication Reviews (SMRs), particularly for patients with polypharmacy, frailty, or complex long‑term conditions
  • Support medication reconciliation following hospital discharge
  • Provide expert medicines advice to clinicians and patients
  • Contribute to long‑term condition management within your scope of competence
  • Support repeat prescribing processes, medication queries, and prescribing safety checks
  • Promote evidence‑based prescribing and contribute to audit and quality improvement work
  • Implement local and national prescribing priorities (e.g. ICB initiatives, QOF, IIF)
  • Review high‑risk medicines and support appropriate monitoring
  • Deliver patient‑facing clinics (e.g. hypertension reviews, inhaler technique, medicines counselling), depending on experience
  • Work collaboratively within the MDT, including supporting trainees and junior colleagues
  • Opportunity to be involved in practice‑based projects, including research and innovation initiatives

About the Candidate

You will be a proactive, patient‑focused clinical professional who enjoys working as part of a team and is motivated by improving systems, services, and outcomes. You will be comfortable with change, keen to develop others, and enthusiastic about using new approaches to optimise medicines management and patient care.
 
Essential criteria: 
 
  • Registered Pharmacist with the GPhC
  • Strong clinical knowledge with a genuine interest in primary care
  • Sound understanding of medicines optimisation principles
  • Excellent communication and interpersonal skills
  • Ability to work both autonomously and collaboratively within an MDT
  • Independent Prescriber qualification (or actively working towards)
  • Experience in primary care settings (e.g. GP practice, PCN, ICB role)
  • Familiarity with primary care systems such as SystmOne, AccuRx, or similar

Desirable criteria: 

  • Experience or interest in research within primary care
