About the Role

Plowright Medical Centre has a great opportunity for a practice nurse to join their busy practice, covering 25 to 30 hours a week, to care for a stable population of appreciative patients who still value general practice.

The main responsibilities of the role include:
  • Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
  • Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met.
  • General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
  • Managing and treating long-term conditions, in line with national and local policies and the practice needs.
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
  • Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
  • Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
  • Meeting the needs of patients presenting for opportunistic wound care.
  • Hypertension review and management.
  • Running asthma, COPD and diabetes clinics and reviews.
  • New patient and health checks.

About the Candidate

All applicants should have the following qualifications, registrations, skills and experience:
  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Ideally having completed the CNO002 Discovering a Nursing Career in General Practice module or the Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) however we would still be interested in those applicants with equivalent experience or qualifications.
  • Competence to undertake long term condition reviews ideally asthma and COPD, holding relevant certificates.
  • Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
  • Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work.  The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
  • Knowledge, and experience of its application, in supporting long-term conditions.
  • Insight and understanding of current issues in nursing and the wider NHS.

About Us

Plowright Medical Centre is conveniently situated within 30–50 minutes of both Norwich and Kings Lynn, surrounded by picturesque villages, beautiful countryside, and great beaches. We are a friendly, forward-looking team working in a well organised practice with a list of around 7,000 patients. The practice is semi-rural with a branch surgery in the nearby village of Necton. At the same time as being traditional, relaxed, supportive and friendly, we are also highly organised and efficient, with tremendous support teams, this enables the GPs to concentrate on the work for which their skills are best utilised. 

We are part of the Swaffham and Downham PCN and have full support and access to a diverse healthcare team including, health and wellbeing coach, mental health practitioners, physiotherapists, social prescribers, pharmacists and practice funded in-house counsellors. We use SystmONE and have a full complement of back office staff removing some GP admin workloads, with a very strong IT, admin and secretarial support. The practice receives a high QOF achievement annually and receives great feedback from our patients.
