About the Role

We are seeking a highly skilled and autonomous advanced nurse practitioner (ANP) to join our team at Corringham Health Centre within the SLH and Corringham PCN. We can be flexible with what days and hours you work, so this can be negotiated, although we are looking for the ideal candidate to work 3 days minimum out of our estate at Corringham Health centre. The role is not offered remotely.   

The successful candidate will deliver high-quality, evidence-based clinical care across a wide range of acute and chronic conditions. Operating within defined professional boundaries, the ANP will independently manage patient care from initial consultation through to diagnosis, treatment, and follow-up.

The post holder will work collaboratively across the six GP practices in our PCN, supporting clinical teams while promoting patient self-management, lifestyle improvement, and holistic, patient-centred care. This includes leading initiatives that reduce reliance on pharmacological interventions and enhance patients’ quality of life.

The post-holder will:

  • Conduct comprehensive assessments, formulate differential diagnoses, and implement evidence-based treatment plans.
  • Manage patients presenting with undifferentiated and complex health needs, both acute and chronic.
  • Provide expert clinical leadership and subject matter guidance in the management of long-term conditions.
  • Triage and prioritise clinical needs, including urgent or emergency situations, and initiate appropriate emergency care when required.
  • Request, interpret, and act upon diagnostic investigations (e.g., pathology, imaging).
  • Independently prescribe and review medications in line with national guidelines and clinical protocols.
  • Promote medication adherence, providing education on pharmacological and non-pharmacological treatment options.
  • Perform minor procedures and wound care within the scope of competence.
  • Maintain accurate, contemporaneous clinical records in accordance with legal and organisational requirements.
  • Adapt communication styles to meet the needs of patients and carers, including delivering complex or sensitive information.
This role offers an excellent opportunity for an experienced and driven ANP to make a meaningful impact within a progressive, patient-focused PCN.

About the Candidate

The successful candidate may come from a range of professional backgrounds. As an experienced healthcare professional, educated to master’s level, they will have developed the skills and knowledge to allow them to take on an expanded role and scope of practice caring for patients. They will demonstrate flexibility in attitude and behaviour and be keen to undertake new ways of working and delivering healthcare, committing to under-taking the professional development required to adapt to changing circumstances.

Qualifications, skills and experience required:

  • Professional registration with relevant professional body: NMC, HCPC or GPhC
  • Post-graduate qualification in specific speciality, or equivalent experience
  • Independent prescriber 
  • Advanced clinical assessment skills
  • Excellent communication skills
  • Ability to perform in stressful and complex situations
  • Sound clinical reasoning and decision making
