North Watford PCN are seeking a Social Prescribing Link Worker to join their team on a part-time basis.
This role empowers people to take control of their health and well-being. A referral to a non-medical ‘link worker’ is designed to support patients in being able to take a holistic approach, connecting people to community groups and statutory services for practical and emotional support.
Duties can include;
- Provide personalised support to individuals, their families, and carers to access community-based activities and support
- Develop trusting relationships by actively listening to individuals concerns and motivations
- Co-produce a simple personalised care and support plan and introduce people to appropriate activities, groups, and services
- Help people identify the wider issues that impact on their health and well-being such as debt, poor housing, being unemployed, loneliness and caring responsibilities
- Forge strong links with local VCSE organisations, community, and neighbourhood level groups, utilising their networks and building on what’s already available
- Work on your own caseload, updating relevant parties as required and referring the patient back to the GP or other suitable health professional if required
