About the Role

Stevenage Healthcare are looking for a highly motivated and experienced general practice nurse on a part-time basis, hours to be discussed at interview. This is an exciting opportunity to take a proactive clinical role in supporting patients with long-term conditions within a supportive and progressive primary care network. 
 
Duties will include;
  • Support housebound patients with a range of long-term condition reviews.
  • Assess and examine patients presenting with minor illnesses (e.g. sore throat, earache, urinary tract infections, rashes, colds, minor injuries).
  • Provide level-1 wound management in our leg ulcer service.
  • Take detailed histories and perform focused clinical examinations.
  • Advise on self-care, over-the-counter treatments, and symptom management.
  • Recognise red flags and refer more complex or severe cases to GPs or secondary care when needed.
  • Educate patients about illness prevention, symptom recognition, and when to seek further help.
  • Promote healthy lifestyles and provide guidance on smoking cessation, weight management, immunisation, etc.
  • Liaise with GPs, pharmacists, and other members of the primary care team.
  • Maintain accurate and timely patient records in line with NMC standards and local policies.
  • Document assessments, treatment plans, and follow-up advice clearly.
  • Stay updated with NICE guidelines, local protocols, and national healthcare policies.
  • Participate in audits, quality improvement initiatives, and professional development.

About the Candidate

All applicants should have the following qualifications, registrations, skills and experience:
  • Registered member of the Nursing and Midwifery Council
  • A strong track record of performing Long Term Condition reviews
  • Experience and training in minor illness assessment and management
  • Experience of delivering LTC health checks for housebound patients. 
  • Excellent communication and interpersonal skills, with the ability to work well within a team and support fellow colleagues (clinical and non-clinical)
  • Commitment to ongoing professional development and high-quality patient care

About Us

Stevenage Healthcare/Stevenage South PCN is made up of local practices who are commissioned to work together to provide a range of healthcare services to a population of 60,000 patients across Stevenage. The core network practices include Bedwell Medical Centre, King George and Manor House Surgery, Knebworth & Marymead Medical Practice and Shephall Health Centre, however other practices may join the PCN in the future.  The PCN is governed by a PCN board, with representatives from each practice, who provide oversight of the service and approval for strategy and future PCN plans and objectives. SSPCN have a fantastic work ethic and strongly believe in supportive teamwork.
