About the Role

Ongar Health Centre are looking for a motivated and enthusiastic advanced nurse practitioner to join our friendly team of 4 GP partners, 1 advance nurse practitioner, 4 practice nurses, 2 HCAs and a large administrative team. Hours and days to be discussed but one working day must be a Wednesday.

Duties and responsibilities:

  • Practice independently in accordance with the NMC code of conduct and with reference to practice guidelines, appropriately referring to GP’s.
  • Deliver a high standard of patient care, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.
  • Assess, diagnose and treat patients’ conditions, initiating direct referrals as appropriate.
  • Provide expert advice to patients, carers and colleagues.
  • Provide the opportunity for you to develop innovative concepts, models, methods and practices to help deliver improved primary care to our practice population.
  • Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills.
  • As an independent prescriber: to prescribe safe, effective and appropriate medication as defined by current legislative framework.
  • Lead and under research and audits.
  • Provide leadership, education and training to different healthcare professionals, staff and students.
  • As a vital part of our multidisciplinary team, you will have the opportunity to support and care for a diverse patient group
  • Ensure continuation of own personal and professional development facilitated by utilisation of clinical supervision networks.
  • Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to general practitioners as necessary.
  • Support practice to achieve QOF
  • Participate in the administrative and professional responsibilities of the practice team.
  • Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.
  • Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines within the practice, with appropriate regard to confidentiality.

About the Candidate

The candidate will demonstrate critical thinking in the clinical decision-making process, will need to be an Independent Nurse Prescriber and have a consultation and assessment/diagnostic qualification.

  • You will be a Registered General Practice Nurse (currently registered with the NMC).
  • You will hold a recognised ANP or ACP qualification and work as an Independent Prescriber.
  • You will be able to work with a high degree of autonomy and be confident in leading in a variety of settings.
  • Skilled in reviewing, diagnosing and treatment of Minor Ailments
  • Your leadership of others will also enable you to use and further develop your teaching and mentoring experience/qualifications.
  • You will be forward thinking and strive to develop innovative ideas and new ways of working, that address the health needs of the practice, whilst motivating and encouraging others to do the same.
  • As an excellent communicator and collaborator, you will embrace opportunities to work and liaise across multi-professional and agency boundaries in the interests of improving patient outcomes.
  • In addition to your highly developed clinical skills, your approach in the role will be consistently compassionate and respectful placing a high value on the dignity of patients, relatives, carers and colleagues.
  • Compassionate, empathetic approach and demonstrable kindness in all aspects of your work as well as valuing the dignity of our patients, relatives, carers and colleagues.
  • Knowledge and application of supporting long-term conditions.
  • Insight and understanding of current issues in nursing and NHS.
