About the Role

Vida Healthcare have an exciting new opportunity for a clinical pharmacist to join their pharmacy team. This position is offered on a full-time basis, however part-time can be considered. 
The successful applicant will be working onsite with patients across our four locations with cross-site cover required.
 
Duties will include the following;
  • Work as part of a multi-disciplinary team, patient facing and remotely, to clinically assess and help treat patients using expert knowledge of medicines for specific disease areas.
  • Provide pharmaceutical consultations to patients with long term conditions as an integral part of the multidisciplinary team.
  • Drawing up structured medication reviews as pharmacist.
  • Completing complex discharge and clinic letters.
  • Improve patient and carer understanding of confidence in and compliance with their medication.
  • Provide advice and answer medication related queries from patients and staff.
  • Organise and oversee the work of the PCN pharmacy team, to help ensure the requirements of the practice are being fulfilled and align.
  • Improve the quality and effectiveness of prescribing through clinical audit and education to improve performance against NICE standards, and clinical and prescribing guidance.
  • Develop yourself and the role through participation in clinical supervision, training, and service redesign activities.

About the Candidate

You will be a qualified pharmacist with strong experience within primary care and have the following skills and qualifications:
  • Previous experience working as a GP pharmacist
  • Possess a MPharm degree or equivalent
  • GPhC registration upon your start date
  • Independent prescriber
  • Experience of SystmOne
