About the Role

Vida Healthcare have an exciting new opportunity for a clinical pharmacist to join their pharmacy team. This position is offered on a full-time basis, however part-time can be considered.

The successful applicant will be working onsite with patients across our four locations with cross-site cover required.

Duties will include the following;

Work as part of a multi-disciplinary team, patient facing and remotely, to clinically assess and help treat patients using expert knowledge of medicines for specific disease areas.
Provide pharmaceutical consultations to patients with long term conditions as an integral part of the multidisciplinary team.
Drawing up structured medication reviews as pharmacist.
Completing complex discharge and clinic letters.
Improve patient and carer understanding of confidence in and compliance with their medication.
Provide advice and answer medication related queries from patients and staff.
Organise and oversee the work of the PCN pharmacy team, to help ensure the requirements of the practice are being fulfilled and align.
Improve the quality and effectiveness of prescribing through clinical audit and education to improve performance against NICE standards, and clinical and prescribing guidance.
Develop yourself and the role through participation in clinical supervision, training, and service redesign activities.

About the Candidate

You will be a qualified pharmacist with strong experience within primary care and have the following skills and qualifications:

Previous experience working as a GP pharmacist
Possess a MPharm degree or equivalent
GPhC registration upon your start date
Independent prescriber
Experience of SystmOne
